First Visit

Your initial appointment will consist of a review of your medical and dental history. An opportunity to discuss your concerns, hopes and dental desires with the doctor, a complete examination including an oral cancer examination and a full periodontal evaluation. Any necessary dental films can be taken at that appointment. Please assist us by providing the following information at the time of your consultation:


  • Your referral slip and any x-rays if applicable
  • A list of medications you are presently taking
  • Your current dental plan information if applicable


If you have dental insurance, bring your current insurance card. If you don’t have one, we will need the subscriber number, group number, and phone number. This will save time and allow us to help you in the processing of your claim.




All patients under the age of 18 years must be accompanied by a parent or guardian at the consultation visit. Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.).




If your dentist or physician has taken x-rays, please have them forwarded to our office as they may be helpful in our diagnosis. A digital radiograph can be emailed to us. If additional films are necessary, they can be taken at our facility.
